Can you add Gift Aid?

If you are a UK taxpayer, we can, as a charity reclaim from the Inland Revenue a further 25p of Gift Aid for every £1 you give, which redirects tax that you are paying in the current tax year to benefit the club. You only need do this once.
Gift Aid Declaration
I want to Gift Aid my eligible donations, and any I make in the future or have made in the last four years, to Lampeter Music Club, Registered Charity No 1051509.
I am a UK taxpayer and understand that if I pay less Income Tax and/or Capital Gains Tax than the amount of Gift Aid claimed on all my donations in that tax year it will be my responsibility to pay any difference.
I will tell you if my circumstances change and I no longer pay tax, if I change address, or if I wish to cancel this declaration.
